LLM SEO: How AI Assistants Decide Which Practice to Recommend

LLM SEO is making your practice easy for the engines behind ChatGPT, Gemini, Claude and Perplexity to find, understand and recommend. When a patient asks one of them for the best med spa in Roseville or a podiatrist near Sacramento who treats heel pain, the assistant runs a web search, reads what it finds about nearby practices and writes an answer that names a few. This article explains how that choice gets made and what moves it.

Brand Lift 360 provides AI search visibility (AEO) for medical and aesthetic practices in Roseville and Greater Sacramento. We check these answers for clients with dated screenshots, so the signals below come from watching real results, not theory.

What an LLM Is, in One Paragraph

A large language model (LLM) is the software behind an AI assistant. It has read a huge amount of the web, so it has general knowledge, but for a local question it doesn’t rely on memory. It searches, reads the pages it finds, pulls out facts and writes an answer. Your job is to make sure the pages it reads say clear, consistent things about your practice.

What Happens When a Patient Asks

  1. The assistant searches. “best med spa in Roseville” becomes a handful of web searches.
  2. It reads the results. Usually the top pages: Google Business Profiles, directory lists, review sites, a few practice websites.
  3. It extracts facts. Practice name, location, treatments, providers, reviews, anything that answers the question.
  4. It writes an answer. Practices with the clearest, most consistent facts get named. Practices it can’t pin down get left out.

That last step is why two practices with similar quality can get very different AI visibility. The one with clear facts in the places the assistant reads wins.

The Six Signals That Decide It

  1. A clear one-line description. “Brand Lift 360 is a medical and aesthetic marketing agency in Roseville, CA.” The assistant needs to classify you in one sentence. If your homepage opens with a slogan instead, it guesses.
  2. One page per treatment. A page that answers “what is this, who is it for, what does it cost, who performs it” gives the assistant something to quote. A single services list doesn’t.
  3. Provider pages. Assistants connect a named surgeon or injector to a procedure and a location. A practice with no provider pages is harder to recommend for a specific treatment.
  4. Consistent facts across the web. Your website, Google Business Profile, directories and social profiles should agree on name, specialty, address, phone and providers. Disagreement lowers the assistant’s confidence.
  5. Reviews, recent and specific. Review count, recency and whether reviews mention treatments by name all feed the answer.
  6. Structured data. Code on your pages that labels your organization, services, providers and FAQs for machines, so nothing has to be guessed.

What We Saw in Sacramento

In October 2026 we asked ChatGPT five questions a Sacramento practice owner would ask, such as which agency a med spa should hire for SEO and who can help a practice show up in AI Overviews. The practices and agencies that were named had three things in common: a specialty stated in one sentence on the homepage, a city in the page title, and listings on directories that repeated the same facts. The ones left out described themselves vaguely or had no directory presence at all.

It worked the same way for our clients’ patients. An allergy and asthma practice was first in the captured Google AI Overview for “best allergy clinic in san mateo,” and a plastic surgery practice is named in Google’s AI answer for its signature procedure. Both have treatment pages that answer the question and provider pages that tie the surgeon to the procedure. See the case studies.

Questions Practice Owners Ask

Does an AI assistant read my website?

Usually yes. Most assistants run a live web search for local questions, then read the top results, including your treatment pages and Google Business Profile. If your site blocks AI crawlers or hides its facts in images and scripts, the assistant may skip it.

Can you guarantee ChatGPT will recommend my practice?

No. AI answers change by question, location and date. We fix the signals that make a recommendation more likely and capture dated screenshots before and after so you can see the change.

Which assistants matter most for patients?

Google’s AI Overviews and AI Mode reach the most people, because they appear inside normal Google searches, and Google's guidance on AI features in Search confirms they draw on the same pages it already ranks. ChatGPT is next. We check ChatGPT, Gemini, Google and Perplexity.
